ULTRA™ FAMILY Gift-with-Purchase Promotion - T&Cs



Promotion Period

1 October to 31 December 2020


Eligibility

a) FREE GWP Gift eCapitaVoucher $20 with purchase of:

  • 4 boxes of ULTRA™ spherical contact lenses; or
  • 2 boxes of ULTRA™ FOR ASTIGMATISM contact lenses; or
  • 2 boxes ULTRA™ FOR PRESBYOPIA contact lenses

How to Claim

(b) Submit a photo of the valid receipt via email to B+L at ULTRA@bausch.com, along with NAME, TEL and NRIC/FIN* (last 4 characters).

*Passport no. (last 4 characters) may be used as an alternative to NRIC/FIN, for claim submission.

B+L reserves the rights to amend any Terms and Conditions without prior notice. Submission of claim implies automatic acceptance of all terms and conditions of the Promotion. In the event of dispute, B+L’s decision is final.



Terms & Conditions

  • ULTRA™ Family Gift-with-purchase (GWP) Promotion is open to all residents of Singapore.
  • Promotion is valid with purchase of 4 boxes of ULTRA™ spherical contact lenses; or 2 boxes of ULTRA™ FOR ASTIGMATISM contact lenses; or 2 boxes ULTRA™ FOR PRESBYOPIA contact lenses.
  • Applicable for purchases made between 1 October to 31 December 2020 (both dates inclusive) at optical shops located in Singapore only.
  • Submission of Claim for Gift can be made only via email to B+L (at ultra@bausch.com), with photo of a valid receipt, Full name, Tel and NRIC/FIN (last 4 characters). B+L reserves the right to invalidate submission that is incomplete.
    *Passport no. (last 4 characters) may be used as an alternative to NRIC/FIN for Claim submission.
  • To claim the Gift, Claimant will be required to install CapitaStar App with a valid account in order to receive/accept the Gift. In the event that claimant is unable to install the CapitaStar App for any reasons, the rights to the Gift will be forfeited.
  • By claim submission, claimant is deemed to have agreed to install the CapitaStar App with a valid account, in order to enable claim and acceptance of the Gift in the form of Digital CapitaVoucher.
  • Gift is strictly not exchangeable for cash, any other types of vouchers or product.
  • Claimant is subjected to verification of eligibility and compliance with all terms and conditions set forth.
  • B+L reserves the rights to verify and invalidate duplicated/multiple claim submission with the same email address &/or receipt.
  • Should there be dispute as to the identity of a claimant, B+L reserves the right, in its sole discretion, to determine the identity of the claimant – who is eligible for the eVoucher under the Promotion.
  • Claimant will receive gift email within Two (2) weeks after submission of claim to B+L (at ULTRA@bausch.com). Pls email to B+L (at ULTRA@bausch.com) if you do not receive the gift email after 14 days from claim submission date.
  • B+L will send a gift email through the CapitaStar App to each eligible claimant. Each gift email will include a link and a set of eCapitaVoucher serial number with a unique pin, to claim the Gift in the CapitaStar App. Each set of eCapitaVoucher serial number and pin is unique and can only be used once.
  • Claimant may click on the link provided in the gift email to open the CapitaStar App, input eCapitaVoucher serial number and the pin to “Accept” the Gift in CapitaStar App. Alternatively, claimant may enter the voucher serial number and pin directly onto the CapitaStar App to accept the Gift.
  • Email provided to B+L at the time of claim submission shall be used as the registered email address for the gift email.
  • B+L is deemed to have fulfilled its obligation for GWP Promotion once the gift email is being sent out to the registered email address through the CapitaStar App.
  • Upon successful acceptance of the Gift in CapitaStar App, Digital CapitaVoucher of $20 value will be stored in the Digital CapitaVoucher Wallet of the account holder (claimant).
  • Upon successful acceptance of the Gift in CapitaStar App, Digital CapitaVoucher of $20 value will automatically be loaded onto “My Vouchers > eCapitaVoucher”.
  • For enquiries pertaining to accepting &/or redemption of eCapitaVoucher in the CapitaStar App, pls email to capitavoucher@capitaland.com or call +65 6631 9931. Alternatively, pls approach the Customer Service Counter of any participating CapitaLand Managed Malls in Singapore.
  • Digital CapitaVoucher $20 is redeemable at any of the 16 CAPITALAND Malls across Singapore. Refer listings

  • All Gift that remain unclaimed (with status reflected as “To be Accepted”) in the CapitaStar App will be forfeited after 28 February 2021.
  • Decisions of the organizer are final, and no correspondences will be entertained.
  • B+L may, at any time and without notice in its sole and absolute discretion, substitute any Gift with any other item of a similar value without giving notice.
  • Any and all personal data submitted may be collected, used and/or disclosed to B+L and its assigned agencies for various purposes, including to communicate with the claimant for purposes related to the Promotion, and such other purposes B+L may reasonably deem appropriate, or in relation to which the claimant has otherwise provided B+L with consent.
  • The Company will only retain Personal Data for as long as necessary for the fulfilment of the purpose for which it was collected or to which you have given your consent, except where otherwise provided by law.
  • If for any reason any aspect of promotion is not capable of running as planned, including by reason of computer virus, technical failure, or any cause beyond the control of the Company, the Company may in its sole discretion cancel, terminate, modify suspend the promotion.
